SAINT® TMS Pros and Cons: An Honest Look

Written by Neuro Wellness TMS Centers of AmericaMedically reviewed by Daniel Pietras, MDPublished November 3, 2026Last medically reviewed November 3, 2026Last updated November 3, 2026

SAINT® is fast (five days, not ~6 weeks), targets a spot personalized from your brain MRI, is outpatient with no anesthesia, and showed 79% remission in the controlled trial. The trade-offs are cost from $25,000, limited coverage, the MRI requirement, few verified providers, and long-term data that is still building. This page lays out both sides plainly.

    Key takeaways

    • Pros: 5 days vs ~6 weeks, personalized MRI target, outpatient, no anesthesia, FDA-cleared, 79% remission in the controlled trial.
    • Cons: self-pay from $25,000, limited coverage, needs a brain MRI, few verified providers (many patients travel), common mild side effects.
    • A lower-cost 5-day program is cheaper and easier to find, but it’s off-label with no personal MRI target — SAINT® results don’t transfer.
    • We verify your benefits for free; we never say “covered by insurance” about SAINT®.

What are the pros and cons of SAINT TMS?

SAINT®’s strengths are speed (five days, not ~6 weeks), a target personalized from your brain MRI, outpatient delivery with no anesthesia, and 79% remission in the controlled trial. The trade-offs are self-pay from $25,000, limited coverage, the MRI requirement, and that long-term follow-up data is still building. Individual results vary, and no outcome is guaranteed.

Pros and cons of lower-cost 5-day programs vs SAINT®

Some clinics sell a 5-day “accelerated TMS” week for less. The real advantage is price and availability: it’s cheaper and easier to find. The real drawback is that it isn’t SAINT® — these programs usually skip the brain MRI, use standard machines with an off-label schedule, and their own providers describe them as untested in the SAINT® clinical trials.

The schedule can be copied. The personalized MRI target, the FDA-cleared software, and the 79% remission result cannot. A cheaper week is not a discount on SAINT®; it’s a different treatment. See the full SAINT® vs accelerated TMS comparison.

Who SAINT® is (and isn’t) a good fit for

SAINT® may suit adults with major depressive disorder who haven’t improved on antidepressant medication, who can’t commit to six weeks of daily visits, or who want a target personalized from their own brain scan. It may not suit patients who can’t safely have an MRI, who have implanted metal near the head, or whose priority is the lowest price regardless of protocol.

Your psychiatrist decides whether SAINT® is appropriate after a full evaluation. Check whether you may qualify or call 954-827-9793.

Is SAINT® TMS Right for You?

Tried antidepressants without enough relief? SAINT® TMS is an FDA-cleared, fMRI-guided treatment for adults with major depressive disorder who have not improved enough with antidepressant medication.

SAINT® uses a personalized brain target and delivers treatment over five consecutive days. It differs from standard TMS in its targeting approach, treatment schedule, and research background. In a small randomized trial, 11 of 14 people receiving active treatment reached remission at the four-week assessment, compared with 2 of 15 receiving sham treatment. Results vary, and a five-day treatment course does not guarantee remission in five days.

How SAINT® Works and How It Differs From Conventional TMS

SAINT® is an FDA-cleared treatment for adults with major depressive disorder who have not improved on antidepressants in the current episode. This video explains how it works, and how it differs from conventional TMS.

Conventional TMS usually means about 30 visits over 6 to 9 weeks. SAINT is five visits over five days.

What makes it different: SAINT is the only FDA-cleared depression treatment that uses an fMRI scan to personalize the stimulation target to your individual brain. A 45-minute scan maps your brain and identifies the specific circuit linked to depression. Treatment is then delivered as ten 10-minute sessions per day, with 50-minute breaks between them, across five consecutive days.

In clinical trials, 79% of participants reached remission, with an average of 2.6 days to remission. SAINT is generally well tolerated; the most common side effects reported were mild headache and temporary scalp discomfort at the treatment site.
