You go to work. That's not the same as being fine.

There's a gap between doing the things and being present for them.

You go to work. That's not the same as being fine.

You get up. You answer the emails. You show up where you said you'd be. From the outside, and possibly from the inside on a good day, nothing looks wrong.

But there's a gap between doing the things and being present for them. You watch yourself perform your life competently and feel almost nothing about it. You're not in crisis. You're just not really here.

Sometimes that's harder to explain than being visibly unwell. Nobody's worried about you. You've become very good at the version of yourself that other people see, and the effort of maintaining it is invisible by design.

Why this version often goes untreated for years

Because you're managing. Because there's no obvious crisis to point at. Because when someone asks how you are, "fine" isn't even a lie exactly — it's just describing the surface.

People in this situation often stay on a medication that isn't really working for a long time, since it's doing enough to keep things stable and changing it feels like more disruption than the problem justifies.

    Years can pass this way.

If you've decided it's worth addressing

SPRAVATO® is one option worth understanding. It's an FDA-approved esketamine nasal spray for treatment-resistant depression, given at a certified center with two hours of monitoring afterward.

The practical shape matters if you're working: sessions start at twice weekly and taper. Each visit runs about three hours, and you can't drive afterward — you'll need a ride every time. Many people schedule late in the day or use intermittent medical leave, and we can provide documentation for an employer if that helps.

It has a boxed warning and real risks. Read them. Then decide whether the conversation is worth having.

    Important Safety Information

BOXED WARNING: SPRAVATO® can cause sedation, dissociation, respiratory depression, abuse and misuse, and suicidal thoughts and behaviors. SPRAVATO® is only available through a restricted program called the SPRAVATO® REMS.

SPRAVATO® can cause sleepiness, fainting, dizziness, spinning sensation, anxiety, dissociation, and breathing problems. Patients must be monitored by a healthcare provider for at least 2 hours after each dose. There is a risk of abuse and misuse, which may lead to physical and psychological dependence. Antidepressant medicines may increase suicidal thoughts and actions in some people 24 years of age and younger. SPRAVATO® is not for use in children.

Do not take SPRAVATO® if you have aneurysmal vascular disease, an arteriovenous malformation, a history of bleeding in the brain, or an allergy to esketamine or ketamine.

Do not drive or operate machinery until the next day after a restful sleep.

The most common side effects include feeling disconnected from yourself, your thoughts, feelings and surroundings; dizziness; nausea; sleepiness; spinning sensation; numbness; anxiety; lack of energy; increased blood pressure; vomiting; feeling drunk; headache; and feeling very happy or excited. If these occur, they usually happen right after taking SPRAVATO® and go away the same day.

Neuro Wellness TMS Centers of America is a certified SPRAVATO® REMS treatment center. SPRAVATO® is only available at healthcare settings certified in the SPRAVATO® REMS program and must be self-administered under the supervision of a healthcare provider.
